The questionnaire

QuestionnaireThe questionnaire has been crafted, by trained auditors to address key issues affecting product safety and legality with questions designed to elicit objective information on which a decision can be made. In many cases answers are supported by the ability to upload documentary information that can be reviewed to verify the answer to the question.

In trials the questionnaire takes between 6 – 8 hours to complete.  Consequently the level of expectation of the questions has been chosen to be sufficiently rigorous to meet required standards of food safety and legality yet avoid the need for unnecessarily complex bureaucracy.

  1. About your business (the scope of certification)
  2. Food safety, risk assessment and control systems
  3. Written procedures (how you do things)
  4. The working environment
  5. Site security
  6. Foreign body control
  7. Staff (training etc)
  8. Hand hygiene
  9. Supplier control (how do you source ingredients etc)
  10. Process and product control (quality testing etc)
  11. Complaints and product related incidents (what happens if things go wrong)
